The triathlon swim is the most chaotic, frenetic segment of the race, and the portion where things are most likely to go wrong. Dying during a triathlon is infrequent, but deaths do happen at a rate of about 1.5 in every 100,000 participants. Nearly 80% of those deaths take place during the swim portion of the event.
